Good to know
- For whomPeople who live in Toronto, are pregnant or parenting a child 3 or younger (children can join until their third birthday), are the child's legal guardian, can have home visits during program hours, and have at least one need, for example: a newcomer to Canada here less than 3 years and having a first baby in Canada, little support around you, a physical or mental health problem that makes parenting or pregnancy hard, questions about your child's health or development, or hard life situations such as homelessness, domestic violence or no OHIP. You do not need an OHIP card.

- Their stepsCall Toronto Public Health at 416-338-7600 (Monday to Friday, 8:30 am to 4:30 pm) and ask to join Healthy Babies Healthy Children. You can refer yourself. Or ask your doctor, midwife, nurse or social worker to refer you with the program's online referral form. Give your consent. A Public Health Nurse and a Family Home Visitor then visit you at home and work with you on your family's health goals. Also ask about Nurse-Family Partnership (first-time parents 24 or younger, less than 26 weeks pregnant, with financial concerns such as low income or Ontario Works) and HARP (pregnant people who are homeless in Toronto and have substance use or acute mental health concerns).
We read all of this on their own website. If something has changed, tell us here. It takes 20 seconds, needs no account, and nothing is deleted: the old fact stays until a person checks yours.
